Dr. Sofia's Mobile Primary Care was born from a simple yet powerful idea: healthcare should revolve around the patient, not the other way around. After years of practice, Dr. Sofia saw a significant gap in care for individuals who found it difficult, stressful, or simply inconvenient to visit a traditional doctor's office. For patients with mobility issues, complex chronic conditions, or busy schedules, accessing quality medical attention was often a major challenge. She knew there had to be a better way.
Driven by a passion for patient-centered medicine and a genuinely compassionate spirit, she created a practice that brings comprehensive healthcare directly to the homes of patients throughout northern New Jersey. With a focus on Middlesex, Somerset, and Bergen counties, our mission is to deliver the same high standard of care you’d expect in a clinic, but with the unmatched comfort and convenience of being in your own familiar space.
A Message from Dr. Sofia
Hi! I am Dr. Sofia Eyakem, DNP, FNP-C
I am a family nurse practitioner. I am a graduate of Rutgers University School of Nursing. Doctoral prepared board-certified family nurse practitioner with specialty experience in both inpatient and outpatient settings. Additional diversified background in wound care, critical care, IV nutrition therapy and pain management. More than two decades of experience in the healthcare industry with managerial role in leadership experiences that include management, scheduling, and oversight of large groups of physicians, nurses and nurse practitioners in both office and hospital settings. Experienced as a clinical preceptor and as adjunct nursing faculty.
I provide compassionate care to all my patients of all ages. I enjoy taking care of patients and their family. I have chosen to become a family nurse practitioner because I can combine nursing and medicine to provide a higher level of care to my patients. As a nurse practitioner I am able to make an impact on my patients’ health through, health promotion, disease prevention, managing acute and chronic conditions and improving patient’s health. Seeing my patients improve physically and mentally is extremely rewarding.

Dr. Sofia brings years of experience to every visit, including two years of specialized work in wound care, ensuring that even complex medical needs can be managed at home.
